Introduces how categories of Peer Support (Mental Health / Substance Abuse / Both) and other identify factors affect the process.  Also introduces Trauma Informed Care and the SOR process, transportation in peer support..

1.3)  HRN PARTICIPANT RELEASE OF LIABILITY (PDF)
This form establishes legal permissions and responsibilities in the HRN Peer Support Team (including the Participant, the Peer Supporter, and Hope Recovery Network).

2.3a) HRN PARTICIPANT EMERGENCY PLAN FORM (PDF)
Useful form to outline a specific plan, important details and contact information in case of an emergency.  Most effective is done early in the peer support process.

2.3b) HRN AUTHORIZATION TO RELEASE INFORMATION (PDF)
HRN Peer Supporters can use this form to document required permissions and specific details of other people in a Participant's support network they want to include in their recovery support.

3.1a)  HRN WELLNESS OVERVIEW (PDF)
Introduces Wellness as defined by the "Global Wellness Institute" including the Wellness Continuum diagram that helps understand the difference between reactive medicine and proactive health.

3.1b)  HRN PARTICIPANT-CENTERED GOAL-SETTING (PDF)
Introduces Whole Health Action Management (WHAM) concepts on how to set "IMPACT" goals. Also outlines actions to meet those goals and how "Participant-Centered" goals are required for effective recovery.

3.2a)  MBTI & TEAM BUILDING WORKSHEET PREP (PDF)
Guides team-building in Peer Recovery support.  Uses Myers-Briggs Type Indicator Personality test/assessment as one example of how to get to know each other to prepare for the HRN worksheet.

3.2b)  PEER SUPPORTER ROLE: ETHICS & BOUNDARIES (PDF)
Defines healthy boundaries for the unique and changing role of Peer Supporters.  Includes a diagram that places Peer Supporters in a line of social roles from medical providers to pure reciprocating friends.

3.3)  HRN PARTICIPANT TEAM PLAN WORKSHEET (PDF)
This worksheet can be used early in the Peer Support relationship to establish communication preferences, goals and ways to address any issues that may arise.

3.4a)  HRN PARTICIPANT ON-BOARD CONTRACT (PDF)
List of 10 principles and guidelines for each member of the Participant Recovery Support team.  This agreement is signed by the Participant and Peer Supporter as equal members of the recovery team.

Illustrates the "Three Strikes" equal participation policy and how to utilize in HRN Peer Support.

List of well-researched 'Substance Abuse and Mental Health Service Administration' SAMHSA brochures, guides, and manuals that are available online that offer information relevant to Peer Recovery Support.

4.3)  HRN PARTICIPANT COMPLAINTS & GREIVANCES (PDF)
Outlines for Participants how to voice a complaint or concern with how Peer Support is going. 'Cause For Concern' can be submitted in paper or done online.  Filing a more formal 'Grievance' will follow Oak House's procedure.

PARTICIPANT TIER SYSTEM (PDF)
Outlines Hope Recovery Network's Tier System, which assess Participant progress to guide resources into 3 categories: Tier 1 (Making Progress) Tier 2 (Maintaining) Tier 3 (Exiting Support).

PARTICIPANT PROGRESS SURVEY (PDF)
This tool helps to measure Participant Peer Support team progress in accordance with the HRN Resource Tier system.  The HRN Resource Tier system is detailed on the back.

HRN PARTICIPANT EXIT SURVEY (PDF)
Documents the end date and important details for the record and ensures Participant has information to restart if relapse occurs. Also gathers useful information of the individual Participant experience to assist with on-going agency effectiveness.

   PARTICIPANT TOOLBOX & WORKSHEETS 

RECOVERY CAPITAL WORKSHEET (PDF)
“Recovery Capacity” is the volume of internal and external assets a Participant has to sustain recovery from substance abuse. This worksheet gathers the levels and helps analyze areas that need additional support.
